POLICY
BOOKING FEE POLICY:
* All appointments will be required to pay a booking fee. Booking fees will be 30% of your scheduled service. Booking fees are non-refundable but may be moved to a different service if rescheduled within the appropriate timeframe {see rescheduling & rebooking policy}
PAYMENT POLICY:
We only accept exact cash, Zelle, Cash app or Venmo for your remaining balance due at the end of your appointment.
LATE/TARDINESS POLICY:
* After 15 mins with no text/call, your appointment will be cancelled and your card on file will be charged the cancellation fee. {see no show policy}
* Taking a client that arrives after 15 mins is at the sole discretion of the owner/service provider.
* If you are late to your appointment, your service will be adjusted to fit the remainder of your appointment time.
* Excessive Lateness: May result in you being unable to book future appointments. If you are 15 mins late to 3 consecutive appointments this rule will apply.
RESCHEDULING & REBOOKING POLICY:
* Rescheduling your appointment from one time slot to another is allowable anytime prior to 48 hours before your scheduled appointment time. Please contact us to move your appointment to another time/date that fits your schedule
* Rebooking occurs when the customer wishes to move their appointment prior to the 48 hour period before their appointment time. The appointment is locked in 2 days before the service time.
CANCELLATION POLICY:
* Life Stylez Esthetics has a 48-hour (2 day) cancellation policy. Your 30% booking fee will be non-refunded and unable to be used on any other service if cancellations occur less than 48 hours.
Sick/Wellness Policy – PLEASE CANCEL YOUR APPOINTMENT at the first sign of symptoms that may indicate that your are unwell or not feeling 100%. This also applies if you are recovering from feeling unwell, please wait at least 7-10 days after you are feeling well to rebook so that we can be sure you are fully recovered. This includes signs of: shortness of breath, fever, cough, temperature increase, body aches/chills, throat soreness or tingle, etc. The owner reserves the right to turn away or refuse to service your appointment should there be any inclination of the aforementioned concerns.
NO-SHOW POLICY:
* 50% of service(s) booked will be charged for no-shows.
* A no-show is a client that does not arrive for their appointment, after 15 minutes beyond your scheduled appointment time with no notification provided of any delay in arrival – you are considered a no show. Your card will be debited 50% of services booked.
* By booking your appointment you are agreeing to the terms outlined above.
NOTIFICATIONS:
* Please be on time for your service.
* Please be courteous and call (leave a voicemail), to 269-601-9987 if you will be late or cannot make your service.
AGE POLICY:
* Minimum age to be serviced is 15 years old. Parents will have to remain on site for their child’s service.
OTHER POLICIES:
* Life Stylez Esthetics reserves the right to refuse to perform a treatment/service based upon professional assessment.
* Life Stylez Esthetics will not be held liable or responsible for any personal items lost or left behind.
* All clients must sign client intake form and waiver in order to be seen.
* No children under the age of 15 years old
* Gratuities in cash are appreciated.
* No refunds; deposits for services are non refundable for any reason
By booking a service with Life Stylez Esthetics you agree to all of the aforementioned terms. Completion of booking services is confirmation of agreement with the above policies and gives permission to Life Stylez Esthetics to draft cancellation or no-show fees.
